Insurance
All patients are required to pay up front during the time of their visit.
Commercial Insurances
Dr. Vivian Fraga is an Out- Of-Network Provider. She does NOT participate with insurances. As a courtesy, the office submits claims to most commercial insurances but cannot guarantee reimbursement.
Medicare
Dr. Vivian Fraga Is Opted-Out of Medicare. Claims CANNOT be submitted to medicare, however patients are given the necessary paper work to submit to their secondary insurance at the end of their visits.
